Our assessment and service planning process is centred on your health, wellbeing, care needs and personal preferences.

Your Support Coordinator will work with you to understand the services available through your package and help you choose the options that best suit your needs.

They will also consider the support you already have in place, including assistance from family, carers, community groups and other service providers.

This means your care is flexible, personalised and designed to give you choice and control over the support you receive, including when and where you receive it.

Yes. We work with you to develop a care package that reflects your individual needs, preferences and budget.

You will be supported to make informed decisions about your care and remain in control of the services you receive.

Support can range from a few hours of assistance each week through to more regular and ongoing care.

Together, we will develop a plan that helps you feel safe, supported and comfortable while remaining in your own home.
